Chief Executive Officer

Carol Turner, RN

Carol Turner has been a Registered Nurse for 33 years and has been in a leadership, clinical operations, compliance, quality role for 30 years. She has served as compliance officer and lead clinical advisor to many different types of healthcare settings from teaching hospitals, to school of medicine clinics to prison clinics, to large home health and hospice agencies as well as small local home health and hospice agencies. She has been part of a leadership team that started multiple home health agencies as well as hospice programs, and a certified compliance professional in all those settings.

Carol states that, “My business philosophy and mission is to give 110% in everything that I do.  To have a heartfelt passion and love for all my co-workers and all those I serve.  To be a servant leader and positive example setter for all who are around me. To continue to grow and learn every day & always be willing to help wherever needed and recognize when I am needed without being asked. To go above and beyond every day in everything I do! Always show happiness, love, compassion, forgiveness, and dedication to all.”

Carol says her “goal has always been and always will be to be a valued member of a quality driven healthcare entity.”  Her 33 years of nursing has been in patient focused companies with a quality and compliance goal priority. 

Carol has had special recognition for having 0 deficiency surveys in many companies she has worked for in her more than 3-decade career.  These include Joint Commission and ACHC and CHAPS Accreditations, as well as initial and recertification state surveys.

When Carol isn’t keeping BeyondFaith Hospice in line, she enjoys spending time with her 2 daughters and 3 grandchildren.  She loves watching football, baseball, or hockey games on TV, as well as watching her grandsons play baseball and her granddaughter do gymnastics. Carol also enjoys doing yard work, crafts, and listening to music, or spending the day at the movie theater with her youngest daughter watching a double or even triple feature late into the night.

Director of Financial Operations

Tracy Collins

Tracy Collins is the Director of Financial Operations, she has over 30 years of background and expertise in financial management with experience in various industries. She began her journey with BeyondFaith in 2008 and has helped to establish other locations across Texas.

She completed her studies in Community School of Bank Management at Texas Tech University.

Tracy shared that she takes great pride in her role, and it is her professional mission to express unconditional love and treat everyone equally.  

Tracy is married to her high school sweetheart Ronnie, they have three sons, two bonus daughters and four beautiful grandkids.  She enjoys spending time with her family and friends either by the pool, at the lake or ranger riding.

Director of Marketing

Leslie Chalmers

Leslie Chalmers graduated with a BBA in Marketing from McMurry University in 1999.  As a student she made the Dean’s list, was active in the Servant Leadership program, was voted Sweetheart of Ko Sari Fraternity, & was head cheerleader of the McMurry University cheer squad.  

Leslie now brings an extensive 22-year background in business & economic development, sales, marketing, public relations, and fundraising from organizations such as Fort Worth Star-telegram, Goodwill West-Texas, Juvenile Diabetes Research Foundation, American Heart Association, Weatherford Christian School, the Jacksboro Chamber of Commerce, the Jacksboro Economic Development Corporation, and most recently accepted the position of Director of Marketing for BeyondFaith Hospice, LLC.  

Over her career, Leslie has raised over $8 million dollars for these different organizations through marketing, donor relations and business development. She is also the owner and consultant of a special event planning & fundraising company, Divine Order Events & Consulting, which educates non-profits on how to execute highly successful events & fundraising campaigns.  

Leslie is passionate about giving back and working alongside organizations to make her community and line of work all it was created to be.  She currently serves in the following roles: Faith Community Hospital Board of Directors - Place 5, Faith Community Health Foundation Board – Board President,

The Vision Group of Jacksboro, Jacksboro Ex-Students Association, & Jacksboro Lions Club just to name a few.  

Leslie was named one of Parker Counties’ Top 40 under 40 and received the Jean Bryant Marketing Award in 2014.  She was nominated, accepted and graduated from the Leadership Texas Class of 2019, an elite program dedicated to ensuring women’s full and effective participation and equal opportunities for leadership at all levels of decision-making in political, economic, and public life.  She was one of only 84 women across the state of Texas selected for the educational and leadership experience that year.   Most recently she was named a Distinguished Alumni in Business from her alma mater McMurry University in Abilene, TX.  

Leslie and her family have given of their volunteer time and efforts through the years to numerous organizations such as the Parker County Livestock Improvement Association, Juvenile Diabetes Research Foundation, Goodwill West-Texas, Concerned Citizens of Jack County, Jacksboro Chamber of Commerce, and The Food Pantry of Jack County.  During her time at Weatherford Christian School, not only did she serve as the school’s Director of Development & Marketing, she also gave of her time as a head coach for the Lady Lion’s basketball team.  During her time as head coach, the Lady Lions won 5 district championships and went on to become state runner-up in 2015 and state champions in 2016.  Leslie was named Coach of Year in 2016 and was asked to coach the TCAF All-Star game. 

Leslie and her husband Stewart returned to Jacksboro in 2017 after meeting there years earlier in high school.  They have two children Rylan, 19, who is a Chemical Engineering major at Texas Tech University and Rachael, 17, senior at Jacksboro High School.  Rachael plans to pursue a degree in forensic psychology at either Texas A&M University or the University of Tennessee.  When Leslie and Stewart aren’t working, helping in the community or following kids’ sports, they enjoy traveling, cooking, being poolside or hiking in Ruidoso.   Leslie attributes all her life’s blessings & experiences to God above and is humbled that she has been used as a vessel to bring honor & glory to His name.

Director of Clinical Services

Kim Mahan, RN

Kim started as a CNA in 1995, then graduated LVN school in 1997.  She went on to obtain her RN license in 2007. She received her CHPN (Certified Hospice & Palliative Care) designation in 2014. She has been with hospice for 10 years now.  

Kim says, “I never thought hospice would be the medical arena I would find my niche but being able to care for patients and also their family members at one of the most vulnerable moments in the circle of life has been one of the greatest blessings of my nursing career.”

She has since assisted in the startup of two of our four BeyondFaith Hospice agencies, Graham, and Weatherford. Kim states, “I think we have created some great teams to provide excellent patient care.”

Kim states that her professional mission and business philosophy is to provide excellent patient and family care and support, as well providing the best work environment with support and work/life balance for staff members.

When Kim is not running the Weatherford office and encouraging her team, she and her husband enjoy trying new restaurants, traveling, and visiting their grandchildren. 

Director of Human Resources

Lauren Mahaney

Lauren Mahaney has worked at BeyondFaith previously, on both the Hospice side as well as Home Health. She handled private insurance billing and Medicaid billing along with various Human Resource responsibilities.   Lauren has a combined 10 year’s finance experience, mostly in banking, as well as several years of experience with HR roles & responsibilities.

She currently serves as a trustee for Graham ISD school board.

Lauren & her husband, Jack, have two teenage boys, Tripp and Reno.  They stay busy with sports and hobbies, and their spare time is spent keeping up with their schedules. They also enjoy spending time at the lake and just hanging out with each other.
